OFFICE HOURS

Our office hours are from Monday - Thursday 9:00am - 5:00pm and Friday 9:00am - 1:00pm, except for holidays.

In case of inclement weather or emergencies, our staff will call you to notify you of our closure.

Our office is closed daily for an hour for lunch. You may call our office and leave a message on our voicemail.

NO SHOWS, RESCHEDULES & CANCELLATIONS

If you are unable to keep your appointment, please let us know at least 24hrs in advance to avoid a no-show charge of $75. This also allows us to schedule other patients. The no show fee will have to be paid prior to the next scheduled appointment. If you fail to No Show 3 appointments, we reserve the right to discharge you as a patient. 

APPOINTMENTS

We request that all appointments be made in advanced. Please arrive 15 minutes before your appointment. 

If you are more than 10 minutes late, we reserve the right to cancel your appointment.

Please report any changes that have been made to your last name, address, phone number, or insurance.

If you are a new patient, please ensure that you bring your photo ID and all insurance cards.

COPAYS/BALANCES

All copays are due at time of service. All balance are also due at time of service. If you cannot pay your balance in full, we will be glad to set you up on a payment plan. If your account becomes delinquent, we may forward the balance to our collection agency. If sent to collections more than once, your patient status may become inactive.

All returned checks will be charged a $25.00 NSF fee.

In case of an emergency after hours, please call 9-1-1.

PRESCRIPTIONS

It is your responsibility to notify the office/pharmacy in a timely manner if refills are needed. Approval of your refill may take up to three business days so please be courteous and do not wait to call. Medication refills will only be addressed during regular office hours. If you find yourself out of medications after hours, please notify our office and we will send a message to your provider. 

It is important to keep your scheduled appointment to ensure that you receive timely refills. Repeated no shows or cancellations will result in a denial of refills. Prescriptions for controlled substances will not be refilled by phone and can only be refilled during an office visit during regular office hours.

FORM COMPLETION

Completing paperwork for schools, camps, the Family Medical LeaveAct (FMLA) claims, long-term care, life insurance, disability claims or other purposes may take up to 7 days to complete. Since all forms require our signature, we are personally responsible for the accuracy of the information provided. Filling out forms thus requires careful consideration and considerable amount of our time.

MEDICAL RECORD REQUESTS

There is a $25.00 fee (CASH ONLY) to request medical records. Once a medical records release form is filled out, we allow up to 7 days to get your documents prepared. We will notify you when they are ready and they may be picked up at our front desk. 

INSURANCE

We participate with a majority of insurances, including but not limited to Aetna, Aetna Better Health, All Savers,  BCBS, Carefirst, Carefirst Community Health Plan, ChampVA, Cigna, Humana, Maryland Medicaid, Maryland Medicare, MD Physicians Care, Mutual of Ohama, Priority Partners, Tricare, UMR, United Health Care, United Health Care Community Plan, WellsPoint.

IT IS YOUR RESPONISIBILITY TO MAKE SURE WE ARE IN NETWORK WITH YOUR INSURANCE. IT IS ALSO YOUR RESPONISBILITY TO BE AWARE IF YOU HAVE A DEDUCTIBLE AND TO PAY IF A BALANCE INCURS.
